Hossein-Ali Nayyeri presided over the Tehran 'Death Committee' that in summer 1988 ordered mass executions of political prisoners pursuant to Ruhollah Khomeini's fatwa — an event the UN has classified as crimes against humanity and genocide. He is explicitly named in Khomeini's written decree as the Sharia judge responsible for the Tehran operations. He personally told Ayatollah Montazeri on 15 August 1988 that 750 prisoners had already been executed in Tehran. The committee he chaired — alongside Morteza Eshraqi, Ebrahim Raisi (deputy), and Mostafa Pourmohammadi (Intelligence Ministry representative) — interrogated and executed prisoners in minutes. He was subsequently appointed Deputy Chief Justice of Iran's Supreme Court (1989–2013) and has headed the Supreme Disciplinary Court for Judges since 2013, facing no legal consequences.
